Why would anyone walk around for 25 years with that thing hanging down? Why wouldn't his colleague or wife or friend or someone say, "Dude, did you know you've got this enormous alien-like bulge of flesh hanging off your abdomen? Might want to get that checked out."

Yeah you get mentally ill people who just have phobias about seeing doctors. There are plenty of women who only show up to get their breast cancer treated when it is bigger than a golf ball and/or eroding the skin. It's denial and mental illness.

Interesting that the patient is Romanian and sounds like the doctors talked him out of surgery. I think that illustrates the way physicians in other countries don't have any incentive to help people with something like this. In the USA I'm pretty sure he gets operated on and it gets written up as a case report where the patient was actually helped.

I love NEJM's images in medicine. It seems to fill our needs for unique and cool diagnoses on a routine basis, something that doesn't happen in routine practice. In residency I used to look for really cool cases. My interns hated me--and probably still do--because I would wander the halls of the hospital looking for cool cases, then I'd call the attending and ask if my service could also follow. I remember one day having over 40 on our list with half of them being ICU or Cardiac ICU patients and my interns begging me to close the service. Being out of residency has left a big void.
