Obama got the Affordable Care Act passed. That is more long-lasting and substantial than anything Biden has accomplished.
That's all he got passed with a veto-proof supermajority in the Senate and control of the House and it wasn't popular at the time and has since been severely gutted. Remember that they did get a version passed in the Senate that couldn't pass the house. Then Kennedy died and they lost their supermajority when Massachusetts elected a GOP senator for the first time in forever...just because the ACA that was passed was not palatable. Then they finally got it all done through the reconciliation process.

I hear that, but in what ways has the ACA really been gutted? What major parts of it have changed? As far as I know, the changes to insurance company regulations, medicaid coverage, etc. have stayed the same.

The major premise of the law was it would
require people to be insured by fining them if they weren’t. There is no fine so people are not compelled to be insured. But you are correct that a number of other provisions (the ones people like) remain.
