    This is so damn frustrating!

    My knee is still injured. Went off on a run, feeling good, and then BAM! Mile 3, no go. Sharp pain again. Not sure what it is - ITBS? Patella? Who knows.

    I saw an orthopedist on Wednesday and he said everything looks fine structurally. Gave me some stretches and some ice.

    I guess mile 3 is better than mile .5, which is what happened a couple of weeks ago.

    I've decided I'm not going to lace anything up for another three weeks - not until the end of November. I'm reducing the mileage on my schedule too. I am going to do elliptical/stationary bike as a proxy for running - using time in HR zone to keep the cardio up - in the meantime.

    This is so frustrating.

    I am with you. I haven't run in more than a month now.

    It sucks and I am sorry. I hope you are feeling better soon.
    What's to explain? It's a bunch of people, most of whom you've never met, who are just as likely to be homicidal maniacs as they are to be normal everyday people, with whom you share the minutiae of your everyday life. It's totally normal, and everyone would understand.

      See if you can find an FP who has a sports medicine fellowship. They know more about soft tissue injuries than ortho's(at least most orthos).

      My partner would be a perfect guy for you to see, but he doesn't do house call.

        Nik, I have had both ITBS and Patellar tendonitis. Sometimes the patellar tendonitis occurs on the lateral edge of the patella and when it does it feels very, very similar to ITBS. The difference for me is that if I run on a flat terrain the patellar tendonitis actually goes away for most of the run. If it's ITBS it gets worse and makes it almost impossible to run. There is an easy test for ITBS the name of which I cannot recall but goose could tell you and I'm sure the docs here can as well. You lie on your side, bring the hip into extension; flex your knee (don't let the femur rotate inward). If it hurts it's itbs.

          Injuries suck. I've only once attempted to train for a marathon, and it ended up in knee surgery. Boo!

            Update: things are getting better. My doc recommended motion control shoes (I'm already an established severe pronator) so I picked up some a while ago. I did 40 minutes on an elliptical last night and then half a mile on a treadmill with no pain. Tonight I am going to try to run around my block (it's a one mile run because of the weird shape) a couple of times to see how that works. At this point, I'm just trying to hold onto my cardio. If I can't run a couple of miles tonight I think I will go swim for an hour.

            The missus is not so lucky. She tore her achilles (partial tear) and has been in physical therapy. It is bad news as far as running is concerned. She's trying other methods of getting back into pre-baby shape (4 kids in 10 years isn't easy, but the effects were such that she's trying to become an athlete again more than lose weight). So p90x is on its way to our house.

                It went okay. I didn't push it. I felt something (not pain - maybe the precursor to it) after a good solid mile, so I stopped and went to the gym and did ellipitical. That is progress, though - things are getting better. I am forcing myself to take it slow. I'm treating every week like a rest week now (4 miles X 3 times a week, plus 6 on Saturday, or whatever the equivalent time at the equivalent heart rate is on a machine - works out to about 30 minutes). I no longer feel pain going down stairs, either.

                I did the ITBS test SteelBlue mentioned yesterday, and no pain, so I'm guessing it's not ITBS. A professional has graciously offered me the opportunity for some free advice, and I'm going to take him up on it.

                  Well, I tried to run again tonight. Felt great. You can tell something is going on/has been going on with my knee, but it's not painful anymore. I started off relatively slow, but I was so excited during the last mile (I did 5) that I ran the last quarter mile at sub 5k pace. Runner's high indeed - I screamed loudly with joy when I got home and scared SHW.

                    good for you. It has now reached the point where it cold enough that it is discouraging me from going on runs. I hate winter.

                    So does this mean that everything should be a go for the marathon?

                      I think so. I considered switching to the half but I am pretty set on doing the full. My knee is a bit sore today but not too bad. I'm taking the day off from running but going out for another four tomorrow. If that goes well, I'll try 10 on Saturday. If I can do that, then I'll ease back into training.

                      The more I read up on this, the more I think it's "just" simple ITBS. Symptoms match, pain matches.

                          From what I've read that you've posted, it sounds like it. Its also consistent with your recent training history, setting in during your increased mileage periods.

                          In addition to slowly ramping your mileage back up, don't forget some hip strengthening exercises, in addition to ITB stretches.

                          As to the strenthening exercises, side-lying hip abduction and hip hiking exercises are simple and effective.

                              According to the latest in evolutionary/kinematic research, a lot of knee problems can be explained by poor running form due to running shoes. Have you done much in the way of barefoot/thin-soled running? The idea is that your form will naturally fix itself if you don't have the cushioning, which prevents your feet from feeling the pain of bad form. It makes a lot of sense to me.

                              That having been said, you'd have to build up slowly to avoid stress fracture and to get your foot muscles in shape, which are probably weak if you're like the average American. It also won't help if you've got something seriously wrong with your knee already.
