Who should perform these? If I have a mole that looks suspect (very) should I just go to my family's primary care physician or straight to a dermatologist?

    Go to the derm, IMO.
Ain't it like most people, I'm no different. We love to talk on things we don't know about.

Any family pcp worth his weight should send you to a derm. Gidget went to our pcp for something unrelated but then asked what he thought of a mole on her forehead. He took one look at it and referred us to a dermatologist. The dermatologist took one look at it and then cut 3" out of her head. The lab came back and it turned out to be melanoma. Thankfully just in situ. So yeah, go to a dermatologist.

I agree with those who already posted. If your insurance lets you go straight to the specialist without a pcp referral then that is what I would do.

I just finished a Derm rotation and was pretty surprised about how quick they are to biopsy even mildly suspicious moles. The rational is that it is pretty quick and painless and could save your life.

I don't regret going to my dermatologist. I had two squamous cell carcinomas removed about 2 years ago. Definitely just go if for nothing else some peace of mind.

Slim, what you fail to realize is the RVU from an excisional biopsy. Lesions are a cash cow. I do a ton of derm in my clinic and it pays very well for the level of difficulty and liability of injury.

Maybe it is because I've trained in a rural community where we very rarely refer (cardio referrals are for caths/pace makers, GI for manometry/dilatation, ortho for surgery, etc...) but I think the culture of medicine has left what it should be...see your doctor; if your doctor can't figure it out he/she asks for help.

I wish my Derm was quicker to remove moles and weird looking spots, my torso is covered with them, but he is reluctant to cut out a bunch of stuff unless it meets every criteria he thinks is important. This makes me nervous since I have had melanoma removed, but he obviously removed that as soon as he saw it so I think I should trust him. And maybe I should be grateful that he isn't just cutting every spot out since I am seeing him out of plan and he could likely make some good easy money by giving me more peace of mind. I am pretty nervous, I know, but melanoma is scary, and deadly, I just wish it was reasonable to get every weird spot removed and biopsied.

Get thee to a Derm - About 5 yrs ago my wife dragged me kicking and screaming and they found a mole that was cancerous. They got it before it spread (thank God) I now have a nice four inch scare on my left sholder blade

I may be small, but I'm slow.

Thanks for the advice, everyone. I saw a dermatologist this afternoon and everything seemed to check out fine. The one "mole" I was most worried about the dermatologist said was more a skin tag grown out of control, but almost for certain benign. He went ahead and removed it anyway and will have it tested.

Fortunately (or unfortunately), with other medical expenses we've had this year we long ago met our family out-of-pocket maximum.
