How are these forms handled in your ward?
In our recent stake conference priesthood leadership meeting, several bishops spoke and talked about how they've used the forms and with each of those wards the bishops said they were the ones that sit down with the new/returning member, go over the form with them, and then present it at ward council periodically to track the progress.
Our bishop has asked the EQP, HPGL, and RSP to be the ones that sit down w/the members to go over the form as a type of "progress map". I don't think that is the best way to do it. I don't know yet if I'd say it is inappropriate, but I definitely think that is something the bishop should be taking care of.
