One possibility is that it isn't as quick to make these secret menu item and at a place that promotes speed, time is everything. If they keep their visible menu small, there will have enough straight forward orders to keep things moving.

I suppose. Though I'm not convinced the large and complex menus at Burger King have an overly adverse effect on their wait times.

I am saying that other fast food places have longer wait times and seem to be doing just fine for it.

I doubt it has hurt I/O sales. They've been able to create an incredible following and the secret menu is partially responsible. It serves as a reward to longtime, loyal customers that they're able customize their food in a way that the casual customer can't. The brilliance of the secret menu is that it's well known, but I/O hasn't had to spend a cent advertising it.
