      Looks like a smashed finger with underlying nailbed injury. In kids there is a risk for an open fracture through the growth plate of the finger bone. If not treated appropriately there is a high risk for infection.

            We have a winner. Her middle finger was caught in the sliding van door last night. I dressed it with gauze and medical tape, but I couldn't really see the extent of the injury. When I opened it up this morning, I realized I needed to take her to the doctor.

            When the primary care physician saw it, she said it needed x-rays because of potential damage to the bone, and that there could be damage to the nail bed.

            The ER doc said he couldn't close the wound since it had been over 12 hours, and they didn't want to trap any bacteria in. After consulting with a hand surgeon, he did place a single stitch just to keep the flap from causing problems. He ordered antibiotics (IV because of some existing issues). We're supposed to change the dressing daily, and not wrap it too tightly.

            Had I taken her in last night (like my wife wanted), I would have avoided a couple steps of trauma for my daughter, and the would have been able to close the wound better, which may have led to a better recovery.
