It will require a longer stay in Mexico but you could just have her drink the water. If she keeps doing that the weight will drop off quickly!
We are transporting enough water for a week but only staying 3 days. I've been sick on 3 separate trips to Mexico so I am well-aware of the risks (Mexico City after eating from a street vendor, Acapulco after drinking the water, and Ensenada after stepping foot in the city .
Why not hit up Costa Rica? At least if you are traveling to a shit hole country you can visit Wuap. I think all dirty scalpels are the same no matter what third world country you visit.

I am being completely sincere. I have relatives with horror stories about cut-rate Mexican surgery, but I imagine there are plenty of success cases. I hope everything goes well for your wife.

if the surgeon offers to make you a deal on some ponchos or a bob marley blanket, make sure that you bargain with him.
Man, I got the coolest poncho at Teotihuacan. I still have it. I think I paid $4. I especially loved the guy who sold it to me. Kept calling to me "eh gordo, ven pa' ca". I couldn't resist.
That's why I never understand these strange diets. A friend was doing that paleo stuff because he wanted to lose weight. He's explaining this laundry list of all the crap he can and can't eat, and I said "just eat fewer calories." He was pretty convinced on this thing though.
Of course it's just calories in minus calories out that will determine weightloss. But some people (me) don't have the discipline to count calories. Whole 30 helped me because I didn't need to strictly count calories because the stuff I was allowed to eat filled me up and wouldn't put me in a positive calorie position for the day...especially when coupled with exercise. I have no illusion that Whole 30 is a permanent lifestyle change for me. I've already gone back to normal foods. But maintaining is far easier than losing obviously.
