What is methadone prescribed for? Is it sometimes prescribed for something other than heroine recovery?

    Chronic pain.
At least the Big Ten went after a big-time addition in Nebraska; the Pac-10 wanted a game so badly, it added Utah

It started to be prescribed for chronic pain about the time of the introduction of OxyContin. It was felt to have some positive traits for this malady and compared to the price of OxyContin, it was a bargain. It has some serious drawbacks for chronic pain use with the big one being that the respiratory suppress can far outlast the pain control that it provides. So when you start ramping up the dose and interval to control pain, it dramatically increases deaths from respiratory suppress, especially in those who have sleep apnea.

And I'm shocked at how many PCP's don't know this, yet apparently feel totally comfortable writing for it (not you, of course ). This is all over our literature--is it in yours?

How likely is it that it would be prescribed for chronic pain to someone who has AIDS?

It is written about extensively in many of the throwaway journals, which is what happens to them. THey are thrownaway before reading.

Old prescribing habits die hard. Once in became acceptable to write for it because it was a cheap alternative, it has remained despite drugs like MS Contin and OxyContin now being in a generic form.

I do have a few patients on it, but they are tested for Sleep Apnea, and they are on it for cost reasons only(cash pay)

Very, depending on what kind of pain the patient is in. Chronic painful condition--perfect case. Methadone is an OK drug--it just needs to be monitored closely and written by someone who understands the pharmacology. It's just not safe as a PRN medicine.

you guys have more experience than me, but I thought methadone was used for opiod addiction treatment. I've never seen one on it, accept for those being treated for substance abuse.

I don't know--I've never tried either. I have to assume that it's less euphoric than heroine--hence its use in heroin clinics. I also have to assume that it provides some sort of high, based on the number of patients I see who either "lose" their prescription or have it "stolen".

It's somewhat regional, in my experience. I've seen it the most in Utah, less in Michigan, and almost never in Indiana. Jarid?

Typically, it will not cause a high, but if you are on in for a prolonged period of time, you will go through narcotic withdrawl if you abruptly stop taking it. It must have some addictive potential given that it is a Schedule II narcotic.

I did not see it in Idaho for Chronic pain, but that was prior to the real surge in longacting narcotic use for chronic pain.. It has widespread use in Cedar City though(Thanks Dr. Warner)

I did see one pulmonologist use methadone for end stage COPD, to help reduce the "air starvation" that those patient's experienced. It was really effect for that scenario.

Wait methadone is effective for air hunger? Wouldn't that suppress the breathing reflex? I had air hunger once after a bought of gastritis and GERD and it was not fun.
